We are now seeking to recruit a dynamic and passionate GP Shared Care Practitioner to contribute to the ongoing success of the service. The post holder will have substantial experience of working within a Shared Care setting and in a recovery focused way in order to improve health inequalities within the Borough.

This exciting role will offer the successful candidate the chance to make a lasting difference to the lives of our service users and be part of an integrated service delivery model within Hackney.

The successful applicant will have a  caseload of service users and proactively engage those not currently engaging with the service and work with service users to achieve their care plan objectives. This will include supporting and delivering interventions,  work in partnership with clinical and psychosocial colleagues, liaising with external partners and hold clinics to assess the needs of service users.
